Preparing for a parent’s return home after hospital
A practical checklist for the first days home, from the house itself to the support around it.
Coming home from hospital is a relief, and often harder than expected. The first few days matter most. A little preparation makes them safer and calmer for everyone.
Before discharge
- Ask the hospital team for the discharge plan in writing, including medications and follow-up appointments.
- Ask what help is recommended at home, and whether Ontario Health atHome will arrange any services.
- Find out what equipment is needed, such as a walker, raised toilet seat or shower chair, and who provides it.
- Confirm who to call with questions after hours.
Getting the home ready
- Clear walkways and remove loose rugs and cords.
- Set up a bed on the main floor if stairs are difficult.
- Put night lights between the bedroom and bathroom.
- Stock easy meals and keep everyday items within reach.
The first week
- Keep a simple list of medications and times, and check it daily.
- Watch for warning signs the hospital told you about, and call if they appear.
- Plan extra help for bathing and getting around, when falls are most likely.
- Make sure family caregivers get breaks too.
Private home care can fill the gaps around publicly funded services, from a few hours a day to overnight support. Booking it before discharge means help is there from the first day home.
